Background information

The patient’s active role is key to caring for their health and treating many illnesses. Support for self-management is available from health care professionals. The self-management is planned together with professionals. The patient receives advice related to such areas as medication, nutrition, use of care accessories and independent rehabilitation.

Following the instructions, the patient can manage their own treatment independently but also contact the professionals at any time if necessary. If the patient's state of health or condition changes in an unforeseen way while they are under self-management, the patient should contact the personnel in charge of their care immediately.
