Reports of missing documents

You can report documents issued by the police that are missing to the police licence administration. This applies to passports and identity cards, for example.

You can report other other missing documents either electronically or in person at a police department at the reception of reports.

You can submit the report either electronically or by filling in a form at the police license services.

You can also use an electronic report of an offence to report a lost or stolen identity card, passport or driving licence. Based on the information provided, the police will consider whether a report of an offence or other report will be recorded in the case.

You need to verify your identity before filling in the form to report an offence electronically. Identity verification can be done using online banking codes, an identity card with a chip or a mobile certificate. The report is entered on the electronic report form that opens after identification. The form contains more detailed instructions on how to complete it and the required information.

Submitting a false report to the police is a criminal offence punishable under the Criminal Code.

To whom and on what terms

If a person is submitting an electronic application for a passport or identity card, the system asks to file an electronic report of offence for the previous missing document.

When visiting the police licence administration in person, the customer fills in a lost item report form on the basis of which the official submits a Schengen report to the police crime reporting system and the document is cancelled.
