To whom and on what terms
You are eligible for the service if you need support for your well-being and everyday life, to avoid social exclusion or improve participation, and you do not have a close network to help you cope in everyday life. For example, you may be at a risk of not getting things done, feel isolated or have difficulties managing your day-to-day life.
