After-care supports young adults as they transition to independent living. It is also available to those who are moving home to live with their parents. In addition, after-care helps parents and others close to the young person.

To whom and on what terms
The services are available to you if you are 18–25-years-old and have been taken into care or placed in substitute care for at least six continuous months. Moreover, no more than five years have passed since you were a client of child welfare services.
You are told about after-care towards the end of your placement or when you turn 18. Your social worker in child welfare will apply for the after-care services for you. If you have not been assigned an after-care worker or want to find out whether you are eligible for after-care, you can contact the after-care unit directly. We will find out whether you have the right for after-care and assign a designated worker to you.
