If you are concerned about your memory, contact the assessment of need for care at your own health station.

Call the Memory Clinic if you wish to cancel an appointment booked at the Memory Clinic or to leave a call-back request for your own memory coordinator.

If you are unable to attend, please cancel your appointment well in advance, no later than 3 p.m. on the previous weekday.
