To whom and on what terms
The services are intended for clients of the wellbeing services county’s health services who are over the age of 16. You must have a medical condition that requires hospital-level care but can be treated at home if you are well enough. You are expected to be able to cope with everyday activities independently or with the support of a relative, home care or nursing home. You must be substance-free for the duration of the treatment.
You need a doctor’s referral to access at-home hospital care. If you wish, you can discuss the referral with your doctor. The At-home Hospital will assess whether the services are right for you based on the referral.
