The statement request can be sent to the Employment Fund by email. If your message contains confidential information, use a secure connection. In addition to the statement request, the Employment Fund will also need the application for the summons, defendant’s reply or an overview of the matter, as well as the name of the unemployment fund that paid the benefit.
