Home visits for assessing service needs

The purpose of a home visit is to support the elderly person living at home for as long as possible and also to provide them with information about the services available. A medical certificate is not required for the request, calling the case manager is sufficient.

A visit for an assessment of the need for service is always based on either an urgent or non-urgent concern. The need to assess the need for services may be related to issues concerning the client'shealth, safety in the home, memory or capacity to function independently, or other issues related to living at home.

Based on the information they have received, the service instructors will arrange a visit in the client's home, which is free for the client. Assessments of the client's need for service in their home focus on identifying risk factors that predict impairment of functional capacity (instability of health, abundant medication, poor nutrition, insecurity, lack of social contacts, pain). During the home visit, a home safety assessment is also carried out in cooperation with the client, and the possible need for benefits (care allowance, housing allowance, etc.) or, for example, aids is mapped out. Evaluations are conducted through tests, discussion, and observations. Utilizing the information obtained, an overall picture of the customer's possible individual service needs is formed. The RAI information system is used in the planning, evaluation and development of rehabilitation services for the elderly in proactive outpatient services.

In connection with the service need assessment visit, a care and service plan that promotes and maintains rehabilitation is drawn up together with the client. The case manager acts as the person in charge of an elderly person living at home who is not yet covered by other services.

To whom and on what terms

The service needs assessment home visit is offered to people over the age of 65 living at home in the Soite area. Home visits are free for clients and are voluntary.

The service is free of charge.
